Metabolic syndrome, as pre-diabetic status, has great public health burden, especially on the complication of cardiovascular disease and brain damage. The association of metabolic syndrome and multiple risk factors such as obesity, hypertension, dyslipidemia and physical inactivity is clearly demonstrated. Insulin resistance, presented by hyperinsulinemia and metabolic syndrome, were prevalent in general population, and its impact on coronary heart disease and stroke events. There are ethnical differences among various obesity indices, including waist circumference, body mass index and skin fold measurements. In this talk, I would like to present the harm of metabolic syndrome on coronary heart disease and stroke in Taiwan, by applying the data in the Chin-Shan Community Cardiovascular Cohort study and the health examination samples. The prospective cohort study based on community, led by Prof. Yuan-Teh Lee, has provided a unique opportunity to investigate the cardiovascular disease events and associated risk factors among Taiwanese. The cohort was composed 3,602 adults older than 35 years old and was regularly follow-up biennially since 1990-91. The data showed significant role of metabolic syndrome on the risk of diabetes and cardiovascular disease and synergistic effect with impaired renal function on cardiovascular mortality.
