Tailored therapy for adult CHD

  Congenital Heart disease (CHD) is the most common congenital anomaly. The incidence from the reports in recent decade ranged from 7 to 20/1000 live births. With the advances of the treatment of CHD in recent three decades, more and more patients with severe CHD can survive to adulthood. In Taiwan, there are more than 110,000 patients with congenital heart disease at present and about three million of them (26.1%) are above 18 years of age. Most of these adult patients face the problems of no longer able to receive professional medical care after grown up. Their residual hemodynamic and electrophyiological disturbances remain significant particularly after the interim postoperative stage. These patients tend to not adhere to follow up, probably due to insufficient medical advice, and lack of integrated medical team. The Adult Congenital Heart Center (ACHC) of National Taiwan University Hospital was established since November 2008.? The goals of the center are to organize a multidisciplinary care unit, which consists of adult CHD specialists, case managers, pediatric cardiologists, pediatric cardiac surgeons, adult cardiac physicians, obstetricians, rehabilitation specialists, and social workers, and to provide an integrated health care. Then the patients can fully understand their health conditions, receive professional medical advices and individualized treatment plan.
