達文西手術在婦科疾病的運用

  Over the past decades, the minimally invasive surgery (MIS) become a trend in the field of surgery. The laparoscopic surgery boomed in the late 1980s and continued to gain popularity into the 1990s and this MIS have been successfully applied to the gynecological benign and malignancy. For many years, MIS appeared to represent an alternative approach in the gynecological surgery compared to the traditional exploratory laparotomy. Another novel MIS technology called “robotic surgery’ which composed of patient-side cart, computer stand-interface and surgeon console is introduced in the 2000s and provide advantages for the surgeons. It is particular useful in obese patients, long-time surgeries and the operation required extensive suturing. This newly technology is similar or better with previous laparoscopic surgery in shortened hospital stay, less blood loss, less surgical pain, a faster recovery and less scarring.
   Since the approval by the US Food and Drug Administration (FDA) in 2005, robotic surgeries have been widely used in gynecologic field including benign and malignant disorder1. The first case of hysterectomy by robotic surgery was reported in 20022. By 2011, 37% of hysterectomies in United State were performed by laparoscopic and 27% were by robotically. (Intuitive surgical Inc., Sunnyvale, CA, USA). In this chapter, we introduce the procedures of hysterectomy, subtotal hysterectomy and radical hysterectomy using robotic surgery system.
